To secure your child’s space for the upcoming school year, please print, fill out, and mail our application to the Chairperson along with a non-refundable registration check for $50 made out to the Seventh District Recreation Council.
After your application and registration fee have been sent in and your space in the program has been reserved, please print, fill out, and mail the following forms to the Chairperson. Please include your first semester’s tuition payment by check, made out to the Seventh District Recreation Council. All forms are due by July 1st of the school year. Per state law, no child will be admitted to the program without all forms completed and on file. All children must be potty trained to attend.
These forms may be returned to the program no later than the first week of September, before school begins to accommodate doctor’s appointments as necessary.
